Medical Records

It is our responsibility to maintain your medical records and X-rays. Your records are confidential, and will only be released if you sign a release. Please allow 7 days for copies of your records and X-rays. There may be a charge for the preparation of the records. Cancellation Policy

9) Please contact our office 24 hours before your appointment if you need to cancel. We have patients on a waiting list, and we are able to accommodate them if you need to cancel. If something unexpected occurs within 24 hours, please still contact us. You may leave a message after hours. Unfortunately, if patients do not show for their appointment twice without calling, a $40 fee will automatically be charged. We hope that everyone understands that we are trying to best accommodate all of our patients

Referrals

Referrals are required for certain insurances to see a specialist. The patient is responsible for obtaining a referral for the visit. If no referral is obtained by the time of the appointment, a waiver will need to be signed to assure that the referral is obtained, otherwise the patient may be responsible for the cost of the visit.

HIPPA

Metro Footcare complies with all HIPPA guidelines. Each patient will be offered current HIPPA guideline, and asked to sign a waiver that the guideline have been offered.

Co-payment

Co-payment is expected at the time of service, or a $5.00 service fee will apply.

Medical History

We require that our patients fill out a complete medical history form. This includes current medications, allergies, previous surgeries, social history and family history. Occasionally patients do not understand why all of this information is necessary. Obviously, we need to know the medical history, current medications and allergies before any prescriptions can be written. We also take personal and family history into account when planning the best treatment plan. The more information that is provided helps with a quick, accurate diagnosis and a tailored treatment plan for the individual patient.
